Position Overview:
  • The Care Navigators are responsible for a variety of functions related to remotely interacting with members of the client’s community and assessing members of the community for risk based on daily self-assessment.
  • Their primary function will be conducting phone interviews.
  • Will work under the guidance of the leadership of the Public Health  Operations Unit, and will report directly to the Supervisory Care Navigators.
 
Responsibilities:
  • Communicating with individuals, ensuring all interviews are guided by standard protocols and include providing disease-specific information; assessing signs and symptoms and underlying health conditions; discussing symptom onset to determine window period for contact elicitation and exposure risk for close contacts; discussing work, social, recreational, and community activities to identify others within the client’s community who may have been exposed; eliciting information on close contacts, including names, exposure dates and locating information; and assessing support needs to maintain health and compliance; providing recommendations for quarantine and isolation and reviewing of daily self-monitoring procedures.
  • Conducting remote monitoring and coordinating and ensuring the provision of needed support to individuals.
  • Conducting remote follow-up with individuals attesting to symptoms or exposure to assess status and provide guidance.
  • Utilizing a database/dashboard system to track individuals throughout their journey, coordinating work with the Health Center, Client Affairs, One Medical, Auxiliary Business Services and other client entities.
  • Providing regular updates to the Supervisory Care Navigators and elevating issues requiring additional support to their attention.
  • Performing other relevant tasks as assigned by Supervisory Care Navigators.
  • Work Interactions:
    • The Care Navigators will be remote using a web-based platform, email and telephone to communicate with and assess identified community members, obtain symptom information, refer contacts for testing, and provide instructions for isolation and quarantine.
    • Required to follow all guidance, policies and procedures provided by Client, and will comply with Client guidance regarding confidentiality and the handling of personally identifiable information.
 
Qualifications:
  • Excellent communication, interpersonal, and active listening skills, including an understanding of how to conduct interviews around difficult issues
  • The ability to elicit and manage sensitive information and ensure the confidentiality of those being interviewed
  • Highly detail-oriented with the ability to prioritize needs and troubleshoot
  • Ability to work independently and as part of a diverse team under challenging, time-sensitive circumstances
  • Willingness to take on additional responsibilities as needed to ensure the success of the client’s response.
  • Comfort working with databases and data management systems
  • Ability to learn and adapt quickly to new information and guidance
  • Excellent organizational skills, research skills and good judgement.
  • Ability to interact appropriately and effectively with a wide range of people.
  • Strong critical thinking skills, excellent composition, grammar and public speaking skills.
  • Strong diplomatic customer service skills
  • Access to a telephone, computer, and reliable internet connection.
  • Experience conducting telephone-based or in-person based interviews.
  • Ability to comply with regulations and communicate official information in an appropriate manner.
  • Excellent interpersonal skills to assist distressed individuals from different cultural backgrounds.
  • Proficient in English, additional languages would be advantageous.
  • Proficient in MS Office as well as data entry.
